NEW ID CLINIC IN THE BLOOR WEST AREA

Sometimes the key that opens doors to needed services is simply having valid government ID.

If you want to open a bank account, apply for housing, get income supports, cash a cheque, or see a doctor…you need ID.
Street Health staff host ID Clinics to help people replace ID: once a week at the All Saints Church; once per month at the Regent Park, Queen West and Parkdale Community Health Centres.

In addition to these clinics, we’re happy to announce a new ID Clinic in the Bloor West area:

Sistering
962 Bloor Street West
(west of Ossington Subway Station)

1st and 3rd Monday of each month
11:30am-1:30pm

We can help with the following:

Birth Registration
Canadian Birth Certificate (wallet size)
Record of Landing replacements
Social Insurance Number (SIN) Card replacements
Ontario Health Card, if person does not have an address and ID
**This clinic is for women only**

N.B. -Since theft and loss are common on the street and in the shelter system, we also provide an ID Safe, a safe & secure place to store valuable ID
